In consideration of my being afforded the opportunity to share my stories on
one or more occasions by ReikiHealingStories. By calling the 1-800-348-5981
or 1-214-231-2905 extension 4951# number, I hereby grant and release to
ReikiHealingStories the right to record my voice and all verbal statements
that I make during each interview in digital format, on audio tape and/or
via other electronic recording media. My voice, together with the statements
that I make during each interview in which I choose to participate, are
collectively referred to herein as my “statements.”
All of my statements will, by virtue hereof, become exclusively owned by
ReikiHealingStories. Consequently, I understand and agree that
ReikiHealingStories and its successors and assigns may:
(i) sell, license, sublicense, reproduce, edit, distribute, exhibit and
otherwise exploit (collectively, “exploit”) my statements (or any of them),
individually, as a compilation with other material or otherwise, without any
compensation or royalties being owed to me;
(ii) exploit my statements in any media, whether now known or hereafter
developed, including, without limitation, in print, microform, audio,
visual, reprographic, electronic, digital, on-line and computer-related
media.
